Shoreline School District Policy
The following is a brief explanation of the policy and procedure for having medication at school. This is based on Washington State law, and the specific policy of Shoreline School District. Full copies are available in the health office for those who would like to read them. If you have any question with these procedures, Please call the school nurse, Debra Eckerlin at 368-4790.
Prescription Medication:
The administration of prescription medication for longer than 15 consecutive days must be authorized, in writing, by a physician or dentist and the parent or legal guardian.
A parent/guardian may authorize, in writing, the administration of prescription medication for up to 15 consecutive school days. The original pharmacy container with a current date, student’s name, name of medication, dosage, and mode of administration, and name of prescribing physician or dentist will serve as the physicians’ prescription
(RCW 28A.210.260 and 28A.210.270)
Over-the-counter Medication:
A parent or legal guardian and physician must authorize, in writing, administration of any over-the-counter medication by school personnel. The medication must be provided in the original labeled container. Unidentified medication may not be administered.
All Medication:
All medication must be brought to school in the original container and brought to the health office. No medication is to be carried by students, even single doses. There are a very few exceptions, with signed permission from the physician and parent in agreement with the nurse.
If you know your student will be requiring medication during the school
year, please call for one to be mailed to you, print out the PDF below or pick it up in the health
office. A similar note from a doctor, signed by the parent/guardian, is also acceptable.
